Locality support services

Who are the locality support services?

We are a social work service, registered with the Care Inspectorate and part of the North Lanarkshire Health and Social Care Partnership.

We offer support that is self-directed to adults who have a disability or long-term condition, in their local communities.

How can you access our services?

To make sure we are providing a service that is personalised and self-directed, we complete an assessment with the person and their carer where appropriate.

This assessment identifies:

  • what is working well in a person’s life - their skills, family and natural supports
  • what assistive technology or aids and adaptations will support greater independence
  • what you would like your life to look like (what your outcomes are and how we could support you to work towards them)
  • whether we are an appropriate service for you and what other supports you might need

We review services annually, to check that the support still fits and that outcomes that have been agreed are being met.

What do we do?

Our main principles are to support people to identify and work towards goals or outcomes, have greater independence and to lead healthier, safer, happier lives.

This might mean support to live independently and will include thinking about technology and equipment to replace direct support from someone who is paid to provide support with tasks. Our support often includes support to learn how to manage money and how to stay safe and well at home.

We support people to be as independent as they can be in their community; to travel independently and to access services, activities and facilities in their local area.

We are well connected and can support people to access services that provide support into employment, groups for people with an interest and opportunities for health improvement.

Some people use our service when they are in transition from school or when their life has changed because of their health. People can feel isolated and disconnected. We provide support to meet other people who share an interest or other people who want to learn a new skill. We help people connect, develop and maintain friendships.
